Essential requirements for the construction

» Medical prescription filled and signed
» Specify who we should invoice, if not the prescribing doctor
» Upper and lower plaster casts or impressions taken with adequate impression materials,
allowing a precise recognition of all teeth, especially the last erupted and of
all anatomical parts of soft tissues
» Habitual occlusion registration
» Construction bite registration in case you require a functional appliance which needs a
specific mandibular position in vertical and anteroposterior direction to transfer on it
» Occlusal splint registration in case you require an appliance with lifted up occlusal
plane to transfer on it
» In case you supply bands for a fixed appliance, they mustn’t be inserted in the impression,
but packed apart, specifying which tooth they belong to
» In case of set-up requirement, specify on which teeth you want it done
and the specific required position
» Inform us of any possible patient allergies to metal or any other material used in the design
and manufacturing of the required appliance
